The paper trail

You’ll need to send us certified copies of some documents to support your name change. What you’ll need depends on why your name changed – but it’s worth noting that in most cases we can only accept documents issued in Australia.

Got married?    

You’ll need one of these:

  • Marriage certificate (Australian-issued only)

  • Change of name certificate (Australian-issued only)

 
Changing to a former name?

You’ll need this:

  • Birth certificate

Plus one of these:

  • Divorce certificate or decree nisi (Australian-issued only)

  • Marriage certificate (Australian-issued only)

  • Change of name certificate (Australian-issued only)

  • Statutory declaration (original document)

 
Changing to a new name?

You’ll need this:

  • Change of name certificate (Australian-issued only)

Once you’ve filled in the form and collected the documents you need, mail them back to us here:

Once we've verified everything, we'll process the changes on your accounts and automatically issue new cards with your updated name on them.
